1. A continuous casting method, comprising the following steps of:
guiding molten steel from an immersion nozzle into a mold;
guiding a slab formed in said mold to a plurality of guide roll pairs;
bulging said slab by providing an increased spacing between a select number of the guide roll pairs to produce a bulged slab with a liquid core;
completing the solidification of a liquid core in a thickness-wise center part of said bulged slab at the position where the bulged slab is reduced, and
reducing the thickness of said bulged slab to a predetermined thickness with a reduction roll pair comprising an upper reduction roll and a lower reduction roll, wherein the uppermost surface of said lower reduction roll is projected a distance higher than a lower pass line for the slab, wherein the projection distance of the uppermost surface of the lower reduction roll from the lower pass line for the slab is based on a bulging thickness of the slab and the projection distance is the same as or not more than a bulging thickness of the bulged slab,
wherein by reducing the bulged slab, a Mn segregation index CC0 in the thickness-wise center part satisfies the relation expressed by the following equation (a) over the width W which is expressed by the following equation (b):

where C: the Mn content (mass %) at the center part in the thickness direction of the slab,
C0: the mean Mn content (mass %) in the slab,
W0: the width of the slab (mm), and
d: the thickness of the solidified shell (the distance between an area of the slab with a solid fraction of 0.8 and a surface layer on a narrow side at the position of thickness reduction) (mm).

1. A method for treatment of cancer comprising administering to a subject in need thereof a therapeutically effective dose of glufosfamide in combination with carboplatin and etoposide.
2. The method of claim 1 further comprising administering rituximab.
3. The method of claim 1 wherein said cancer is a lymphoma.
4. The method of claim 3 wherein said cancer is a non-Hodgkin’s lymphoma.
5. The method of claim 3 wherein said cancer is a Hodgkin’s lymphoma
6. The method of claim 1 wherein said glufosfamide is administered in the absence of mesna.
7. The method of claim 1 wherein glufosfamide is administered for one or more dosage cycles, each cycle comprising infusion in the range of:
a): about 1.5 to about 8.0 gm2; about 1.5 to about 6.0 gm2; about 1.5 to about 4.5 gm2; about 4.5 to about 8.0 gm2; about 4.5 to about 6.0 gm2; or about 4.5 to about 5.0 gm2 or over an infusion period of 1-6 hours once every three weeks;
b): about 1.5 to about 3.0 gm2 or about 1.5 to about 2.0 gm2 over an infusion period of 1-6 hours for three consecutive days (days 1, 2 and 3): every three weeks;
c): about 1.5 to about 2.0 gm2 over an infusion period of 1-6 hours once per week; or
d): about 1.5 to about 8.0 gm2; about 1.5 to about 6.0 gm2; or about 1.5 to about 4.5 gm2 over an infusion period of 1-6 hours once every four weeks.
8. The method of claim 2 wherein said cancer is a lymphoma.
9. The method of claim 8 wherein said cancer is a non-Hodgkin’s lymphoma.
10. The method of claim 8 wherein said cancer is a Hodgkin’s lymphoma
11. The method of claim 2 wherein said glufosfamide is administered in the absence of mesna.
12. The method of claim 8 wherein said glufosfamide is administered in the absence of mesna.
13. The method of claim 2 wherein glufosfamide is administered for one or more dosage cycles, each cycle comprising infusion in the range of:
a): about 1.5 to about 8.0 gm2; about 1.5 to about 6.0 gm2; about 1.5 to about 4.5 gm2; about 4.5 to about 8.0 gm2; about 4.5 to about 6.0 gm2; or about 4.5 to about 5.0 gm2 or over an infusion period of 1-6 hours once every three weeks;
b): about 1.5 to about 3.0 gm2 or about 1.5 to about 2.0 gm2 over an infusion period of 1-6 hours for three consecutive days (days 1, 2 and 3): every three weeks;
c): about 1.5 to about 2.0 gm2 over an infusion period of 1-6 hours once per week; or
d): about 1.5 to about 8.0 gm2; about 1.5 to about 6.0 gm2; or about 1.5 to about 4.5 gm2 over an infusion period of 1-6 hours once every four weeks.
14. The method of claim 3 wherein glufosfamide is administered for one or more dosage cycles, each cycle comprising infusion in the range of:
a): about 1.5 to about 8.0 gm2; about 1.5 to about 6.0 gm2; about 1.5 to about 4.5 gm2; about 4.5 to about 8.0 gm2; about 4.5 to about 6.0 gm2; or about 4.5 to about 5.0 gm2 or over an infusion period of 1-6 hours once every three weeks;
b): about 1.5 to about 3.0 gm2 or about 1.5 to about 2.0 gm2 over an infusion period of 1-6 hours for three consecutive days (days 1, 2 and 3): every three weeks;
c): about 1.5 to about 2.0 gm2 over an infusion period of 1-6 hours once per week; or
d): about 1.5 to about 8.0 gm2; about 1.5 to about 6.0 gm2; or about 1.5 to about 4.5 gm2 over an infusion period of 1-6 hours once every four weeks.
15. The method of claim 4 wherein glufosfamide is administered for one or more dosage cycles, each cycle comprising infusion in the range of:
a): about 1.5 to about 8.0 gm2; about 1.5 to about 6.0 gm2; about 1.5 to about 4.5 gm2; about 4.5 to about 8.0 gm2; about 4.5 to about 6.0 gm2; or about 4.5 to about 5.0 gm2 or over an infusion period of 1-6 hours once every three weeks;
b): about 1.5 to about 3.0 gm2 or about 1.5 to about 2.0 gm2 over an infusion period of 1-6 hours for three consecutive days (days 1, 2 and 3): every three weeks;
c): about 1.5 to about 2.0 gm2 over an infusion period of 1-6 hours once per week; or

b): about 1.5 to about 3.0 gm2 or about 1.5 to about 2.0 gm2 over an infusion period of 1-6 hours for three consecutive days (days 1, 2 and 3): every three weeks;
c): about 1.5 to about 2.0 gm2 over an infusion period of 1-6 hours once per week; or

b): about 1.5 to about 3.0 gm2 or about 1.5 to about 2.0 gm2 over an infusion period of 1-6 hours for three consecutive days (days 1, 2 and 3): every three weeks;

b): about 1.5 to about 3.0 gm2 or about 1.5 to about 2.0 gm2 over an infusion period of 1-6 hours for three consecutive days (days 1, 2 and 3): every three weeks;
c): about 1.5 to about 2.0 gm2 over an infusion period of 1-6 hours once per week; or

b): about 1.5 to about 3.0 gm2 or about 1.5 to about 2.0 gm2 over an infusion period of 1-6 hours for three consecutive days (days 1, 2 and 3): every three weeks;
c): about 1.5 to about 2.0 gm2 over an infusion period of 1-6 hours once per week; or
